Your Rights as a NUTRATIVO Patient
NUTRATIVO is committed to ensuring that every patient receives respectful, high-quality care. As a patient, you have the following rights:
Right to Respectful Care
You have the right to receive care that is considerate, respectful, and delivered without discrimination based on race, color, national origin, sex, age, disability, or any other protected characteristic.
Right to Information
You have the right to receive complete and current information about your diagnosis, treatment options, and prognosis in terms you can understand. You have the right to know the name and credentials of your treating provider.
Right to Informed Consent
You have the right to make decisions about your care, including the right to refuse treatment. You will be informed of the risks, benefits, and alternatives to any proposed treatment before consenting.
Right to Privacy and Confidentiality
You have the right to privacy regarding your medical information. Your PHI will be handled in accordance with our Privacy Policy and HIPAA Notice of Privacy Practices.
Right to Access Your Records
You have the right to access, review, and request copies of your medical records in accordance with applicable law.
Right to Voice Concerns
You have the right to voice concerns or complaints about your care without fear of retaliation. Complaints may be submitted to [email protected].
Right to Continuity of Care
You have the right to receive information about follow-up care and to have your care coordinated appropriately.
Right to Know About Costs
You have the right to receive information about the costs of your care before services are rendered.
Your Responsibilities as a Patient
In return, we ask that you: provide accurate and complete health information; follow your agreed-upon treatment plan; notify your provider of changes in your health; treat all staff and providers with respect; and fulfill your financial obligations.
