Informed Consent for Telehealth Services

By submitting an intake form on the NUTRATIVO platform, you acknowledge and agree to the following:

Nature of Telehealth

Telehealth involves the delivery of healthcare services using electronic communications. You understand that telehealth has potential benefits including improved access to care, but also limitations including the inability to conduct a physical examination.

Provider Independence

All clinical decisions, including whether to prescribe medication, are made exclusively by independent, licensed healthcare providers. NUTRATIVO does not direct or control clinical decisions.

Accuracy of Information

You certify that all health information you provide is accurate and complete to the best of your knowledge. You understand that providing false information may result in inappropriate treatment and could be harmful to your health.

Voluntary Participation

Your participation in telehealth services is entirely voluntary. You may withdraw consent at any time by contacting [email protected].

Risks and Benefits

You understand that, as with any medical treatment, there are potential risks and benefits associated with the treatments available through NUTRATIVO. Your provider will discuss these with you as part of the consultation process.

Emergency Situations

Telehealth is not appropriate for medical emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately.

Medication Risks

Prescription medications carry risks and potential side effects. Your provider will review these with you. You agree to report any adverse reactions to your provider promptly.

Right to Refuse Treatment

You retain the right to refuse any recommended treatment at any time without affecting your right to future care.
